Employee Benefits*
First Suburban National Bank is proud to offer its full time employees a comprehensive benefits package as outlined below. Employees are eligible for benefits after successful completion of the 90 Day Introductory Period. Some part time employees are eligible for some but not all of the benefits outlined below.
Medical Insurance
HMO and PPO plans offered through Blue Cross Blue Shield. Employees pay 30% of premiums. The effective coverage date is the first day of the month after 90 days.
Dental Insurance
PPO plan offered through Delta Dental. Employees pay 30% of premiums. The effective coverage date is the first day of the month after 90 days.
401K
Provided through ING Financial Services. The bank matches 50% of the first 3% of salary contributed. Matching funds are vested at 20% per year of service up to 100% at 5 years. Contributions may begin the first of the month after 90 days.
Flexible Spending Accounts
Medical and Dependent Care Flexible Spending Accounts are available through Documentation Administration Corporation. Enrollment is annual. You do not have to be enrolled in FSNB's medical or dental programs to participate.
Short and Long Term Disability
Provided through Principal Financial Group at no cost to employees. Coverage initiates the first day of the month after 90 days of employment.
Life Insurance and AD&D
Provided through Fort Dearborn Life at no cost to employees. Life insurance is 2 times salary with a maximum of $300,000. Dependent coverage of $5000 for spouse and $2000 for children is also available at no cost to employees.
Vacation
Vacation is awarded based on job designation and length of service beginning with 2 paid weeks.
Sick
6 paid days.
Paid Holidays
The bank observes 10 holidays and employees are awarded an additional day to use for personal holidays.
